Etymology

心配 is a compound of two kanji: 心 (*kokoro*, heart/mind) + 配 (*hai*, to distribute or arrange). The idea is that your heart is "spread thin" or "scattered" across concerns — a vivid metaphor for the distracted, restless feeling of anxiety. The word entered common usage in the Edo period as a Sino-Japanese (*kango*) compound.

Cultural note

心配 is extremely common in everyday Japanese speech. A classic expression of social care is 「心配しないで」(*shinpai shinaide*) — "Don't worry" — which you'll hear constantly from friends, family, and even shopkeepers. Japanese culture places high value on not causing others to worry (心配をかける, *shinpai wo kakeru*), so reassuring people proactively is considered a social courtesy. Be aware that "shinpai" is not a false friend for any common English word, but English speakers sometimes confuse it with *shimpa* in Korean (심파), which has a different meaning.
